Mr. Daniel S. Flynn
Director of Health and Social Services
Mr. Flynn, or “Dan” as he prefers, is the Director of Health and Social Services for the Congregation of the Passion, Province of St. Paul of the Cross (CPPROV). In this role he, assists Province Leadership in Administrative oversight of Health and Social Service infrastructure and Province Members in coordinating and managing their changing health and social service needs including. Work activity may involve:
- Assessing/evaluating Members’ comprehensive health and social service needs either on-site (in residences, health care treatment settings, or other locations) or via telephonic interviews,
- Working with Members and their immediate supporters (local Community leadership, Provincial leadership, family) to develop, implement and monitor action plans designed to meet Members’ health and social service care needs,
- Directing Province activity regarding issues of sexual misconduct and other Protection and Advocacy concerns with primary responsibility to mitigate associated risk to individuals served through Province ministry, Members of the Province and the CPPROV.
- Identifying available/appropriate health and social service care professionals, facilities and agencies; financial entitlement options; public and commercial funding eligibility alternatives,
- Assisting in developing and maintaining in each Community the health and social service infrastructure that is best suited to ensure the optimal provision of health and social services,
- Coordinating and/or providing education programs and resources including the ongoing development and implementation of clinical and administrative procedures, practices and protocols designed to facilitate enhanced wellness for Province Members by highlighting best practices and current developments in health and social services;
- Being the lead Advocate and Liaison for the Province and/or Member to the cross-continuum, multidisciplinary team of professionals who are chosen by the Member and/or his designated health care proxy (as appropriate) to provide health and social service care on the behalf of the Member,
- Developing open, reciprocal and sustainable channels of communication with these multidisciplinary healthcare teams and organizations including medical professionals (i.e., physicians, nurse practitioners, social workers, therapists, counselors, etc.), funding entities, home health agencies, skilled nursing facilities, pharmacies and community-based service organizations to increase synergy of efforts to manage Members’ health and social service needs,
- Warranting that healthcare and social service delivery is primarily based on the choices and decisions of the Member who has been fully informed of treatment and service delivery options,
- Safeguarding that Members’ rights are respected and that Members understand their corresponding responsibilities related to the provision of safe, member-centered, consensual health and social service care,
- Ensuring that each Member is engaged in care in the least restrictive setting appropriate to meeting their needs within the limits of reasonable accommodation,
- Assuring quality service delivery to Members. Acting as liaison on behalf of Members to ensure Member satisfaction with service delivery and that Providers meet identified clinical outcomes,
- Making recommendations to minimize Members’ safety risk and associated risk to the local Community, Province and Congregation.
- Providing behavioral and mental health consultation, behavioral and/or brief mental health assessment and counseling (as applicable and appropriate) on behalf of Members.
Dan initiated his post-secondary education at Providence College where he acquired a BA in Humanities/Psychology as well as pursuing certification in Soviet Studies while a student at the University of Fribourg, Switzerland. He then completed graduate studies in Rome, Italy obtaining a degree in Theology from the Gregorian University and completing studies in Moral Theology from the College of St. Alphonsus. Dan is a Master in Social Work (MSW-Gerontology) having completed graduate studies in social work at the Southern Connecticut State University. He maintains a license to practice Clinical Social Work. He is also a Justice of the Peace for the State of Connecticut.
In his previous career pursuits Dan, as a LCSW, has provided psychosocial, pastoral and spiritual counseling and, in particular, engages individuals in the application of Cognitive Behavioral Therapy and Mindfulness principles and techniques to assist in the resolution of the negative effects of trauma, abuse, and/or victimization. He is certified as a Clinical Trauma Professional. Dan also worked at Connecticut Community Care, Inc. in home and community-based care management as a Quality Coordinator, a Co-Lead of the Community Passport to Care transitions of care program, a clinical supervisor for the Connecticut Home Care and Money Follows the Person programs. Dan completed formal Care Transitions Program® training and is qualified as a Transitions Coach® to provide the Care Transitions Intervention® . He has been certified to facilitate workshops in the Stanford University Better Health, Better Choices® chronic disease and diabetes self-management programs. Dan served on the Board of Directors of the National Academy of Certified Care Managers. He has also served as the social work representative to the professional advisory committee of Interim Healthcare of Hartford.
Dan has facilitated workshops and retreats on a variety of topics. These include the aging experience, coaching in care transitions, care management and the elderly, ethical issues in care management, end of life decision making, financial entitlements in the care of the older adult, contemplative/mystical spirituality and the spirituality of older adults. He has been published in the Gerontology and Geriatrics Education Journal for research done on behalf of the Association for Gerontology in Higher Education related to distance learning in gerontological education.
